A safe indoor and outdoor environment was not provided for the children. The outdoor play area. The white rock contains smaller rocks presenting a choking hazard. The wooden play kitchen had exposed rusty nails. The blue outdoor planter had sharp edges and splintering wood accessible to children.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In Space 112, small pebbles were in the science center accessible to children. In Space 111, paint daubers with a warning not for children under three (3) was stored in an unlocked cabinet on a shelf less then five (5) feet. In Space 102, small clothespins and thumb tacks displaying artwork were accessible to children.

Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection dated December 3, 2024 was not sent to DCDEE within one week of the approved report.

In Space 110, Room 103, baking soda was located on a shelf lower then five (5) feet in an unlocked cabinet accessible to children.

Standing water was observed in a trash bin on the playground.

Equipment and furnishings were not sturdy, stable and free of hazards. While in Space 101, I observed a stack of cots tilted over in the book area, as I touched them they began to lean towards me falling over.

The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. Two(2) medical action plans on site for medications for chronic conditions were not updated annually. One was provided in July 2022 and the other was provided in August 2022.

A violation was found for which there is no item number. While monitoring in classrooms this morning, it felt cold and children were wearing jackets. The thermostat in Space 101 controls four (4) classrooms and read 57 degrees. This is a violation of Sanitation requirement 15A NCAC 18A .2826(b).

Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. Four staff (hired (2) on 12/6/21, 5/31/22 and 7/25/22) do not have a current staff development plan on file.
