The EPR Plan did not include the location of the Ready to Go File and or the required information. The program’s Emergency Preparedness and Response Plan and Ready to Go File were monitored during today’s visit. It was observed that the Ready to Go File did not contain emergency information for all children currently enrolled in the program.

Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Upon reviewing staff files it was observed that each staff member's file contained both personnel-related forms and medical information.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In the child care space a plastic shopping bag was observed hanging from the handle of the door leading to the outdoor learning environment.

Enhanced staff/child ratios and group sizes were not met. Ms. Hill was observed present with six (6) enrolled children with one child under twelve months old present. The program is licensed to follow enhanced ratios of one caregiver to five children when there is an infant under twelve months present.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A storage shed located in the outdoor learning environment containing hazardous materials labeled Keep out of the Reach of children accompanied by other warnings was observed unlocked.
