All stationary equipment, more than 18 inches high, was not installed over protective surfacing. On the preschool playground, mulch surfacing did not meet the requirements of 6 inches.

All equipment and furnishings were not in good repair. On the preschool playground some of the play structures had chipping paint and rust.

Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. In space 109, a package of baby wipes was stored in the bathroom used by children.

In space 107 a topical ointment expired 2/2024. In space 110 two emergency medications expired 12/2023 and 1/2024.
