A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. A child enrolled on 12/1/25 did not have documentation of receiving the summary of NC Child Care Law.

The activity plan did not provide at least 4 different activities daily listed in GS 110-91(12): art/creative play; books; blocks; manipulatives; and family living and dramatic play, including one of which is outdoors if weather conditions permit. In space 4, the activity plan did not include at least 4 different activities.

A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. On 4/9/26, a staff member handled a 3-year-old child in a rough and inappropriate manner when the staff member grabbed the child by the face, and pushed the child into a windowsill, resulting in an injury to the child's head. Also, on the same day, the same staff member handled a 4-year-old child in a rough and inappropriate manner by aggressively grabbing the child by one arm, shoving the child onto their nap cot, standing in front of the child not allowing the child to stand up, and aggressively threw the child onto the ground, because the child would not stay on their nap cot.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Space #8 had two sun blocks and one bug spray that were in aerosol dispensers.

Children were not adequately supervised at all times. A staff member that cares for children between the ages of two years and three years of age was observed on video footage from August 25, 2025 looking at a cell phone while a child two years of age has a plastic bat in his hand and is observed running into a water fountain and then he was observed heading towards the staff member.
