Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. In Space #3, an electric fan was on a table to the left of the housekeeping center, accessible to children. The fan was plugged in and running and was not mounted or fitted with a mesh guard. In Space #5a/b, an electric fan was on top of the cabinet used to store mats for naps. The fan was plugged in and running and was not mounted or fitted with a mesh guard.
