Equipment and furnishings were not sturdy, stable and free of hazards. On the toddler outdoor play area concrete blocks installed to hold the poles for the shade had sharp corners exposed, the wood borders had splintering and a metal bench had sharp edges with rust exposed under seat slats.

Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The Fire Inspection conducted March 13, 2025 was not submitted within one week of the inspection visit.
