Equipment and toys were not in good repair and developmentally appropriate. The outdoor learning environment was monitored, and it was observed that there was a small plastic Little Tykes sliding board present with a small chip and a crack approximately 5 inches long on the lower right corner.

Developmentally appropriate activities for the ages of children in care, were not provided as planned on the written schedule and activity plan. While conducting the walk-through of the childcare space it was observed that the posted lesson plan was not current, as it was dated January 2025.

All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. It was observed that there were exposed tree roots in the area used by children for outdoor learning activities.

Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. It was observed that the posted attendance had not been completed for the day and there were four (4) preschool children present.

Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. During the walk-through multiple bottles of household cleaning supplies were observed stored in the lower area of an unlocked closet on the path of travel between the child care space and the family room.
