All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One (1) aerosol can of Sanis Micro Metered Air Freshener, one (1) bottle of Air Purpose Peppermint Sage All Purpose Cleaner, and one (1) container of Disinfex Disinfecting Wipes, all with multiple warnings and all accessible to children, were on the shelf the bathroom used by children between Space #4 and Space #5.

Center staff did not comply with the safe sleep policy. In Space #1, one (1) infant, eight (8) months of age, had a visual sleep check conducted on November 20, 2025, at 2:00pm and not again until 2:20pm. In Space #1, one (1) infant, ten (10) months of age, had a visual sleep check conducted on November 4, 2025, at 12:50pm and not again until 1:10pm, on November 12, 2025, at 1:20pm and not again until 1:45pm, and on November 21, 2025, at 12:30pm and not again until 1:10pm.

On the van used to transport children, the upholstery was torn, exposing foam on the seat of the left seat on the back row, on the left armrest on the left seat on the second row, and on the left armrest of the middle seat on the first row.

In Space #2a, one (1) aerosol can of Chase’s Home Value Spray Disinfectant, with multiple warnings, was in the unlocked closet in the classroom, accessible to children. In Space #4, the classroom used by children enrolled in Space #2a and Building 2 Space 1a/b, one (1) bottle of Lysol Brand New Day Clinging Toilet Bowl Cleaner, one (1) bottle of Clorox Clean Up Cleaner and Bleach, and one (1) bottle of Windex Original, all with multiple warnings, were in the unlocked closet in the classroom, accessible to children. In Space #6, one (1) bottle of All Purpose Peppermint Sage All Purpose Cleaner, with multiple warnings, was on the diaper changing table, accessible to children.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Space #1, one (1) sleeping infant, three (3) months of age, had a visual sleep check documented on May 5, 2025, at 9:10am and not again until 9:30am, on May 20, 2025, at 1:17pm and not again until 1:37pm, and on May 21, 2025, at 11:39am and not again until 11:56am. In Space #1, one (1) sleeping infant, nine (9) months of age, had a visual sleep check documented on April 23, 2025, at 11:05am and not again until 11:30am, and on May 13, 2025, at 12:00pm and not again until 12:30pm. In Space #1, one (1) sleeping infant, ten (10) months of age, had a visual sleep check documented on May 22, 2025, at 12:57pm and not again until 1:15pm. Per your facility’s safe sleep policy, a visual sleep check must be conducted and documented for each infant every fifteen (15) minutes at a minimum.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In Space #3, six (6) syringes with liquid inside, labeled with a staff member’s name, were in an unlocked refrigerator. One (1) bottle of prescription Methylphenidate Tablets were in an unlocked box on a shelf five (5) feet five (5) inches from the ground in the restroom used by children between Space #4 and Space #5.

Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current fire inspection available for review was dated August 30, 2023. The previous fire inspection was conducted on June 30, 2022. The fire inspection was not submitted to DCDEE within one week of the inspection. The fire inspection report was emailed to the DCDEE Child Care Consultant on September 13, 2023.
