All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #4, I observed Arm & Hammer handsoap near the sink, accessible to children that says " keep out of reach of children".

All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. In space R3, I observed peeling paint on the blue cubbies, and through out the building, I observed peeling paint on the baseboards of the classrooms. I also observed peeling paint on the handrails on the steps near the preschool classrooms.

Children were not adequately supervised at all times. At 4:02pm a toddler exited out of space #R2 and roamed the hallway alone, the toddler then entered into space #R4 (the classroom for three-year-olds). The classroom was empty due to the children being outside on the playground. A parent then entered the classroom at 4:06pm to pick up her child in space #R4 and noticed the toddler alone. She then picked up the toddler and returned him back to his classroom.
