A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. A staff member handled children in a rough and inappropriate manner. While in the staff member's care, two children, both two years of age, sustained serious bruising to their upper arms and one had additional bruising on the underarm area/chest as well. In addition, on August 19, 2025, a staff member pinched a two-year-old child in the shoulder. On the same date, the staff member roughly pulled a two-year-old child by the leg then pushed the child backwards on a cot by the child's shoulder.

The morning of 12/31/24 face to name sheets reported thirty-three (33) children ages two (2) to seven (7) starting at 6:52 am. Children were moved to other classrooms based on their age reducing the number of children cared for to twenty-seven (27) ages two (2) to three (3) for the reminder of the day. The ratio based on the youngest age in the combined age group, which was two (2) years old, was not met day. This is out of compliance with the voluntary enhanced ratios permitted on the license.

Center staff did not comply with the safe sleep policy. Center staff did not comply with the safe sleep policy. Due to an air conditioning malfunction the air conditioning could not maintain the required temperature of 68F-74F. During today's observation the temperature in the classroom was 78F and the children were present. On June 25, 2024, and June 28, 2024, the temperature exceeded the Safe Sleep temperatures, and the children were present per the Enviornmental Health Specialist.
