Various completed incident reports were not filed in the applicable child's file. They were maintained in the incident log binder instead.

Incident logs were not completed and maintained as required. An incident report completed and signed on 11/13/2024 was not monitored logged onto the center's incident log. Ms. Demerson added the missing incident to the log during the visit.

The temperature in a room where infants aged 12 months or younger where sleeping exceeded 75 degrees. The infant room/Space #1 temperature was monitored at 79f instead of maintaining at 75f or under. The temperature was manually altered by the caregiver during the visit and the temperature was later monitored at 75f.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Three (3) infants were in their crib, one was sleeping, nothing was documented on the safe sleep chart. When I asked how long the infant had been sleeping, the teacher stated since 10:00, it was 10:46. She stated the other infant just woke up and had been sleeping for about an hour. She stated that the third infant had just been put in the crib.

All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. The EPR plan was completed, but had not been reviewed with the staff.

One (1) infant was lying awake in a crib, the staff member stated the infant had just woken up. The Safe Sleep Chart had last been documented at 9:45, it was now 10:20 and nothing else had been documented.

Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. An employee who started employment March 19, 2024 did not have documented that the required orientation had been completed.

The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. The center did not have a developed ITS-SIDS policy posted in the infant room. In the older infant space, not in use, there was a sample ITS SIDS policy that was not customized.
