All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The mulch around the play surface measured under three inches for some places, mainly under the slide.

Prior to employment a Criminal Background Check was not completed. One staff was alone with the children and did not complete the process to get her out of state CBC to get her NC CBC completed.

Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu posted was applesauce, french toast, and water. Many children had cheese crackers and water.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Space three had a missing cover by the shelf by the entrance near the hallway.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On July 23, 2024, a staff member touched a ten-year-old child’s face during verbal redirection in order to get the child’s attention. In addition, the same staff member used an inappropriate tone when verbally redirecting school aged children.

Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Consultant did not receive the required fire or sanitation inspection, one week after the inspection was conducted.

The outdoor premises were not clean, drained and free of litter and hazardous materials grass and other vegetation in a manner which does not encourage vermin. Litter: Tissue, trash and other debris was located in several spaces on the playground.

When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. Space one did not have a log completed.
