Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space #5, I observed small pieces of crayons in a container on a shelf accessible to children ages two years old. On the OLE for toddlers, I observed beach balls (some were deflated), accessible to toddlers.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #4, I observed an electrical outlet that was not in use use, without a safety cover.

All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. In space #2, I observed peeling paint on the wall.

In space #4, I observed a caregivers bag underneath the sink not properly locked that contained personal creams and medication.

All walls and ceilings including doors and windows were not kept clean and in good repair. In Space #1 (one) the floor underneath both handwashing sinks was observed to be not in good repair. In Space #2 (two) the walls near the cribs had hole in the walls from the removal of equipment and peeling paint. In Space #3 (three) the wall by the block area was not in good repair.

A violation was found for which there is no item number. In Space #3 (three), while washing my hands I noticed that the water was extremely hot. Ms. Dukes was able to check the temperature (thermometer read 115.3 (one hundred and fifteen point three) degrees which exceeds the maximum temperature of 110 (one hundred and ten) degrees. Refer to 15A NCAC 18A .2801 (39) in the North Carolina Sanitation Laws.
