During the visit on August 17, 2026, I observed there was one staff member caring for three children, one year of age, three children, three years of age, and one child, four years of age for the majority of the visit. On August 4, 2026, a total of five children from ages four months to two years of age were present from 8:30am to 4:35pm with one staff member. On August 5, 2026, a total of six children from ages four months to two years of age were present from 11:35am to 4:45pm with one staff member.

In space #1, the designated office space there was one aerosol can of Lysol disinfectant spray, one aerosol can of Off bug repellant, one aerosol can of air freshener, two aerosol cans of room spray, one spray bottle of lens cleaner, and one tube of contact cement that had hazardous warnings listed on the labels and were all accessible to children since the office door was open during the visit. Per sanitation requirement .2803 e(6), Handwashing procedures shall include the following steps: (1) using liquid soap and tempered water; (2) rubbing hands vigorously with soap and tempered water for 15 seconds; (3) washing all surfaces of the hands, to include the backs of hands, palms, wrists, under fingernails, and between fingers; (4) rinsing the hands under tempered water for 10 seconds; (5) drying the hands with a paper towel or other hand-drying device; and (6) turning off faucet with a paper towel or other method without re-contaminating hands. One (1) employee in space #4, did not use a paper towel to turn off the faucet when she finished washing her hands. She used her wet hand to turn the faucet off.
