Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. Children could potentially have access to a lawn mower, propane tank, a power tool (for cutting, and a leaf blower). These items were all located when you walk outside to go to the playground.

Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily. Provider did not have any evidence of the required document posted in the area that the infant slept.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. Documentation was not kept on file, provider stated that she gave the documentation to the infant's parent.
