In Space #1B, four (4) plastic packages containing diapers were stored under the unlocked diaper changing table and accessible to one (1) infant and one (1) child two years of age.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space #1a/b, one (1) container of Clorox Healthcare Bleach Germicide Wipes and one (1) bottle of Clorox Cleanup Disinfectant Cleaner with Bleach, both with multiple warnings, were in an unlocked cabinet above the right handwashing sink. In Space #1a/b, one (1) container of Clorox Healthcare Bleach Germicide Wipes with multiple warnings was on a shelf above the paper towel dispenser to the right of the window.

Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. There was no playground inspection form on file for November 2023 or December 2023.
