A safe indoor and outdoor environment was not provided for the children. In the Infant classroom, it was observed that both cribs utilized for Emergency Evacuations were positioned in areas where they were blocked by other items including shelving and materials utilized by children during daily play.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Also, while conducting the walk-through of the program three (3) loose laundry detergent pods were observed being stored in an open container on a shelf attached to the wall less than five feet from the ground.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In the Toddler classroom, a small pink pouch with a zipper containing three bottles of eye drop solution was observed being stored on a shelf underneath the changing table.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In both the Toddler and Two-year-old classrooms plastic bags were observing hanging from hooks less than five feet from the ground.

Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. . One bottle was not labeled in the infant room.

Balloons were accessible to children. When I entered the facility there were balloons in the front lobby for the grand opening of the facility. Ms. Labinski removed the balloons and threw them away.

Center staff did not comply with the safe sleep policy. In the infant room there was a mobile hanging from one of the cribs.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In the infant room two infants were napping, there was no record of Safe Sleep charts being completed since April 8, 2026.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member did not have current First Aid on file.

Center did not maintain a record of daily attendance In the infant room attendance had not been documented for this week. Ms. Labinski and Ms. White stated that they also used an app to track attendance, however there were discrepancies between the paper attendance and the app attendance.

Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. The sign on the door only stated no smoking, we discussed that it is also needed to state no tobacco use on the sign as well.
