On multiple occasions, a staff member failed to provide adequate supervision when the staff member used a personal mobile phone while responsible for a group of two-year-old children. Additionally, on March 12, 2026, the staff member failed to provide adequate supervision when the staff member remained seated at a child sized table. The staff member was not engaged in an activity with children and failed to move around the classroom.

Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The annual fire inspection was due no later than 10/13/2023. The annual inspection was not obtained until 12/01/2023.
