Bed, cribs, playpens, cots or mats were not placed at least 18" apart or separated by partitions when in use. Cots in space 1, space 2, and space 4 had cots placed 16 inch apart or less in some areas.

The center did not maintain records as required in rule, and/or were not made available to the Division for review. The Outdoor playground inspection could not be located but there was a trend that it was done the same day as the fire drill.

Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. Cardboard blocks in the three year old classroom had holes in them and tattered.
