Openings in equipment, steps, decks, and handrails were not smaller than 3 1/2 inches or greater than 9 inches to prevent entrapment. There is a section of the fence on the left hand side closest to the house that has an opening that is measuring between 4-5 inches between the mesh and fence post.

Openings in equipment, steps, decks, and handrails were not smaller than 3 1/2 inches or greater than 9 inches to prevent entrapment. The protective covering used to cover the entrapment hazards (spaces between posts larger than 3.5 inches) on the railing on the deck and fence surrounding the fence around the outdoor play area was very loose and pulled away from the railing and fence.
