This requirement was not met as evidenced by: Based on interviews and documents recieved it was found that there were rodent and rodent droppings found in the facility. This poses a potential health and saety risk to children in care.
ALLEGATION(S): Licensee does not live in the home. INVESTIGATION FINDINGS: On June 17th, 2024 Licensing Program Analyst (LPA) Kassandra Medrano, conducted a subsequent site visit to the facility to deliver investigation findings. LPA met with Zhonglin Wang, Licensee, and Michael Shen, Facility Representative the purpose of the visit was explained. Michael was present and translated for Zhonglin as licensee speaks only Mandarin. Based on LPAs observations, record reviews, and interviews which were conducted. The allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Exit interview conducted and a copy of this report and appeal rights were reviewed and provided to licensee, Zhonglin Wang. Unsubstantiated Estimated Days of Completion:

(a) Prior to making alterations or additions to a family child care home or grounds, the licensee shall notify the Department of the proposed changed, including, but not limited to, the following:(6) Any change from an area of the family child care home previously identified as "off limits" to an area where care and supervision will be provided to children in care. This requirement was not met as evidenced by: Based on interviews and observations licensee used back yard portion that was only marked for crossing through. This space is marked off limits. This poses a potential health and safety risk to children in care.
d) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing, or volunteering in a licensed facility:(3) Request and be approved for a transfer of a criminal record exemption, as specified in Section 102370.1(p), unless, upon request for a transfer, the Department permits the individual to be employed, reside or be present at the facility. This requirement was not met as evidenced by: Based on observations and interviews, Yunhan "Rachel" Shen is present in the facility daily. This poses an immediate health and safety risk to children in care.
