Napping equipment: Floor mats used for napping shall be: Maintained in a safe condition with no exposed foam. This requirement was not met as evidenced by: LPA Matos observed twelve napping mats in Classroom 1 during today's inspection that have exposed foam. Y.V., director, removed the mats from Classroom 1 during today's inspection. This presents a potential risk to the health, safety, or personal rights of children in care.

Purpose of today's investigation: Deliver investigation findings. Based on interviews, observations, record reviews, and evidence gathered during the investigation process, it is concluded that although the allegations noted on this complaint may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. The allegations are thus UNSUBSTANTIATED. Exit interview conducted and report was reviewed with the Director, Y.V. No deficiencies issued. ALLEGATION(S): Facility is not meeting day care child's needs INVESTIGATION FINDINGS: Licensing Program Analyst (LPA) Mel Matos conducted an unannounced follow up complaint investigation and met with Claudia Albornoz, director. Purpose of today's follow up complaint investigation: deliver investigation findings. The investigation of the complaint allegation listed in this complaint was conducted by LPA Mel Matos. Based on the available evidence, including observations of the Facility, documents reviewed, and interviews completed for the complaint investigation, it is concluded that although the allegation noted on this complaint may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
