PERSONAL RIGHTS: The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations. This requirement is not met based on it being reported facility was having problems with fleas inside resulting in some children getting flea bites. This poses a potential health and safety risk.
ALLEGATION(S): PERSONAL RIGHTS: Staff engaged in inappropriate interactions with child in care. INVESTIGATION FINDINGS: Licensing Program Analyst (LPA) Andrea Medlin met with facility representative for this conclusionary complaint visit. Purpose of visit explained. Initial complaint investigation was conducted on 5/24/2023 by LPA Andrea Medlin; LPA obtained a children's roster and staff roster with contact information. This complaint investigation was handled and investigated by our Investigations Bureau (IB) and they conducted all subsequent visits, gathered information, conducted interviews, as well as obtained supplemental reports. Based on the Department's Investigation Bureau (IB), it was determined there was a lack of sufficient evidence to support or deny the allegations. Based on this information, the findings of these allegations are unsubstantiated. This report was reviewed with facility representative and a copy of this report must be made available for public review upon request. Notice of site visit posted and shall remain posted for 30 days. (1) Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in one out of five persons which poses a potential health, safety or personal rights risk to persons in care.
