(k) The licensee shall maintain documentation of criminal record clearances or criminal record exemptions of employees, volunteers that require fingerprinting and non-client adults residing in the facility. This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the section cited above. Licensee stated that her adult son (S2) resides in home and only helping her today to take care of day care home children, which poses an immediate health, safety or personal rights risk to persons in care.

CCR 102416(c) · Type A

(c) The licensee and other personnel as specified shall complete training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above. Licensee does not have current pediatric CPR/First aid certificate on file. Licensee stated that her assistant(S1) and present Adult(S2), who is helping her today to take care of the day care home children do not have current pediatric CPR/First aid certificate on file, which poses an immediate health, safety or personal rights risk to persons in care.

CCR 102425(j)(1) · Type B

The provider shall supervise infants while they are sleeping and adhere to the following requirements: The provider shall physically check on the infant every 15 minutes. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above. Licensee stated that she does not have record of infant safe sleep log for 15 minutes check up for child (C1), which poses a potential health, safety or personal rights risk to persons in care.

HSC 1596.8662(b)(1) · Type B

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited. Licensee, aisstant (S1) and Adult (S2), are missing current Mandated Reporter Training (AB1207) certificates. Licensee stated that Adult(S2) is only helping her today to take care of day care home children,which poses a potential health, safety or personal rights risk to persons in care.

CCR 102417(g)(4) · Type A

Operation of a Family Child Care Home: (g)The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not be limited to: (4) Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. This requirement is not met as evidenced by: Based on observations, , the licensee did not comply with the section cited above. LPAs observed multiple kind of knives in one of the top drawers in kitchen area, cleaning supply inlcuding lysol wipes and spray under the kitchen sink area, kitchen counter, bathroom and living room areas and two sharp tools in the backyard of the home, accessible to children, which poses an immediate health, safety or personal rights risk to persons in care.

HSC 1596.803 · Type B

Child day care facility licenses; fees assessments; additional fees; use of revenues collected; grounds for forfeiture or denial (e) The failure of an applicant for licensure or a licensee to pay all applicable and accrued fees and civil penalties shall constitute grounds for denial or forfeiture of a license. This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the section cited above, toal amount of fee owed of $420 annual fees has not been paid, which poses potential health, safety or personal rights risk to persons in care.

Safety precautions shall include but not limited to: (4) Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above, LPA observed Lysol wipes and detregents on floor and reach of children. which poses an immediate health, safety or personal rights risk to persons in care.

CCR 102416(c) · Type B

(c) The licensee and other personnel as specified shall complete training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866. This requirement is not met as evidenced by: Based on record review, Licensee did not comply with the section cited above. Licensee does not have proof of current Pediatric First aid and CPR on File, which poses a potential health and safety risk to children in care.
