101229: Responsibility for Providing Care and Supervision: (a) "The licensee shall provide care and supervision as necessary to meet the children’s needs (1) No child…shall be left without the supervision of a teacher…" This requirement was not met as evidenced by: Based on observation, LPA observed two children playing in the far-left corner of the fairy room without 100% direct supervision by Director. During visit LPA also observed Director walking into kitchen area leaving children unsupervised in multipurpose room and fairy room. This poses an immediate health, safety and personal rights risk to children in care.

ALLEGATION(S): Staff used inappropriate forms of punishment with children in care Staff left child unsupervised INVESTIGATION FINDINGS: On July 23, 2026, at approximately 8:50am, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced visit to deliver complaint findings for the above allegations. LPA met with Owner, T.A., and explained the purpose of the visit. Present during today’s visit was Owner and assistant caring for 7 preschoolers. Facility is operating within capacity. LPA conducted a health and safety inspection of the indoors and outdoors of facility. During the course of the investigation, LPA conducted interviews, observations and reviewed pertinent documentation that was received. LPA determined that based on the information obtained there is not sufficient evidence to prove that staff used inappropriate forms of punishment with children in care and staff left child unsupervised. Although the allegations above may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the above allegations are UNSUBSTANTIATED. Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain postings as required will result in an immediate $100 civil penalty. This report is public and can be reviewed. ALLEGATION(S): Licensee handles children roughly INVESTIGATION FINDINGS: On May 27, 2026, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced visit to deliver complaint findings for the above allegation. LPA met with Assistant Director, M.T., and explained the purpose of the visit. Present during today’s visit was Assistant Director and one staff member caring for 14 preschool age children. All staff present have criminal background clearances and associated with facility. Facility is operating within capacity. During the course of the investigation, LPA conducted interviews, observations and reviewed pertinent documentation that was received. LPA determined that based on the information obtained there is not sufficient evidence to prove that Licensee handles children roughly. Although the allegation above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the above allegation is UNSUBSTANTIATED. (1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test. This requirement is not met as evidenced by: Based upon record review, the facility did not test for lead until 4/24/2023. This posed a potential health, safety, or personal rights risk to the persons in care.
