CCR 101220(a) · Type B

(a) Prior to, or within 30 calendar days following the enrollment of a child, the licensee shall obtain a written medical assessment of the child. This medical assessment enables the licensee to assess whether the center can provide necessary health-related services to the child. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in two children (C1 & C4) out of six children do not have physican report in their files, which poses a potential health, safety or personal rights risk to persons in care.

CCR 101216(g)(1) · Type B

(1) Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in one staff (S1) out of five staff do not have healthscreening and TB test done, which poses a potential health, safety or personal rights risk to persons in care.

CCR 101161(a) · Type B

(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in two children (C1 & C4) out of six children were enrolled before they turned 2 years old, which poses/posed a potential health, safety or personal rights risk to persons in care.

ALLEGATION(S): Staff handled daycare child in a rough manner. INVESTIGATION FINDINGS: Licensing Program Analyst(LPA) Mandeep Kaur met with Director, N.K. for an unannounced follow up complaint investigation. Purpose of today's investigation: deliver investigation findings. LPA conducted observations, interviewed staff, children and random parents during the investigation. LPA toured inside and outside areas of the facility during investigation. Based on interviews and observations during the investigation process, it is concluded that although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. CCR 101216.3(a) · Type B

Teacher-Child Ratio: (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement is not met as evidenced by: Based on observations, records review and interview with Director, LPA observed that Staff (S2) was supervising nine(9) children in the playarea without a qualified teacher on 04/15/25 at 11:09AM. Director self-admitted that staff (S2 & S5) do not have evaluated transcripts by approved/authorized department by U.S. department of education. Director stated that Staff (S2) has a degree from Ukraine and Staff (S5) has a degree from Russia. Director stated that Room B (S4 & S5) does not have a qualified teacher present with 10 children during today's investigation.
