Buildings and Grounds: (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.(1)The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as evidenced by: Based on observations and interviews, the staff do not keep the facility free of pests as rat droppings were observed in the classrooms: #2 and #7 under the sink areas and a dead cockroach in a trap in Room #2 under the sink area, which poses a potential risk to health, saftey, and personal rights to the children in care.
ALLEGATION(S): Facility did not maintain the center in a safe, sanitary and good repair to ensure the safety and well-being of children. INVESTIGATION FINDINGS: Licensing Program Analysts (LPAs), Mandeep Kaur and Syhshyan "Shine" Yu met with the Site Director, Megan Eitzen, and explained purpose of visit- to deliver complaint investigation findings. During the course of the complaint investigation, LPA Kaur conducted observations and interviews. Interviews revealed there was a random smell in Room 2 bathroom. There was a urine smell at the start of the school year. Parents had raised some concerns and staff reported that they are getting children potty trained. On occasions, children have accidents and staff clean with paper towels. During this transition, the staff confirmed there was a smell but couldn’t clean with bleach while children are in care. ALLEGATION(S): Child's personal rights are violated by staff member. INVESTIGATION FINDINGS: Licensing Program Analysts (LPAs), Cortney Nelson and Sheena Chin, met with Site Director, Megan Eitzen, and Head of School, Ramyaa Venkatesh, and explained purpose of visit- deliver complaint investigation findings for the above allegation. LPA was admitted into the facility by office staff upon arrival. LPA Nelson conducted interviews with current and former staff members, the Site Director, and children attending the facility. No children interviewed expressed that their personal rights are violated by a staff member. Staff members and the Site Director did not indicate any concerns regarding staff employed at the facility. Only one former staff member expressed concerns regarding a staff member at the facility. LPA observed a classroom on 11/7/2023 and did not observe teachers violating children's personal rights. Based on the available evidence, it is concluded that although the allegation listed above may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. The allegation is thus UNSUBSTANTIATED. As a result of this investigation, no deficiencies were cited. Exit interview was conducted and report was reviewed with Site Director, Megan Eitzen, and Head of School, Ramyaa Venkatesh. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
