ALLEGATION(S): Staff do not provide adequate supervision resulting in inappropriate interactions between children. Director is falsifying incident reports. INVESTIGATION FINDINGS: On 07/08/2026, Licensing Program Analyst (LPA) Kate Huang conducted an unannounced complaint visit to deliver the investigation findings regarding the above allegations. Upon arrival, LPA met with Director S.K. and explained the purpose of the visit. It was alleged that staff do not provide adequate supervision, resulting in inappropriate interactions between children. It was also alleged that the director is falsifying incident reports. During the course of the investigation, LPA conducted unannounced facility inspections on 04/28/2026, 04/29/2026, and 05/28/2026, and observed that student to teacher ratios were in compliance with Title 22 regulations. LPA interviewed the head of school, the center director, and eleven staff members. LPA also interviewed parents. In addition, LPA reviewed the Employee T.E. Report, student injury logs, and children’s records. Unsubstantiated Estimated Days of Completion:

Responsibility for Providing Care and Supervision: No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: one preschool child was left unattended for approx 1 min in Classroom 5 during transition time on 04/19/24. This presents a potential risk to the health, safety or personal rights of children in care.
