Responsibility for Providing Care and Supervision(a)(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: C1 was left unattended in inside Classroom A1 on 2/3/2025, which poses a potential risk to the health, safety, and personal rights of children in care.
101700.3 Lead Testing Written Directives (b)(1) a result with values of 5.5 parts per billion (ppb) or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidenced by: Lead water testing conducted at Stratford School produced Action Level Exceedance of 92.0 ppb for Faucet "S" which poses a potential risk to the health, safety, and personal rights of children in care.
