102416.3(a) Prior to making alterations or additions to a family child care home or grounds, the licensee shall notify the Department of the proposed changed, including, but not limited to, the following: This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as plans for alterations were not readily available, which poses an immediate/potential health, safety, and/or personal rights risk to persons in care.
(a) The licensee shall maintain, in each child's record, the signed and dated notice form required in Section 102419(d). This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as 3 of 4 children's files reviewed by LPA are not complete. One child in attendance during the inspection does not have a file, which poses/posed a potential health, safety or personal rights risk to persons in care.
(e) If no assistant provider is present at a Large Family Child Care Home, then the licensee shall comply with the capacity requirements for a Small Family Child Care Home as specified in subsections (b) and (c). This requirement is not met as evidenced by: Based on LPA observation and record review, the licensee did not comply with the section cited above as Licensee was the only adult present in the home with 8 preschool-age children upon LPA arrival, which poses/posed a potential health, safety or personal rights risk to persons in care.
