1596.8662(b) (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on records review and interview, the licensee did not comply with the section cited above by not having 3 out of 3 staff completed required mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

HSC1597.622 · Type B

1597.622 (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on records review and interview, licensee did not comply with the section cited above by not having required mmuniziations records on file for staff (S1 & S3) on for review: staff (S1) is missing TB test which poses a potential health, safety or personal rights risk to persons in care.

CCR 102359(a) · Type B

Advertisements and License Number: (a) Licensees shall reveal each facility license number in all advertisements, publications, or announcements made with the intent to attract clients. This requirement was not met as evidenced by: Based on the interview with licensee and evidence gathered during the investigation process, licensee did not include facility license number in all advertisements which poses a potential health, safety or personal rights risk to persons in care.

The provider shall supervise infants while they are sleeping and adhere to the following requirements: The provider shall check and document the following: This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above by missing or out of date 15 min. sleeping check records/logs for all infants which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR 102418(g) · Type B

Immunizations: (g) The licensee shall document each child's immunizations as required by the California Code of Regulations, Title 17, Section 6070, and shall maintain such documentation for as long as the child is enrolled. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above that 4 of 5 children in care were missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR102425(j)(1) · Type B

Infant Safe Sleep: The provider shall supervise infants while they are sleeping and adhere to the following requirements: The provider shall physically check on the infant every 15 minutes. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above on missing 15 minutes sleeping check logs for all the infants which poses/posed a potential health, safety or personal rights risk to persons in care.
