101216.3: Teacher-Child Ratio: " (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance..." This requirement is not being met as evidence by: Based on observation, interview, and record review, the facility did not comply with the section cited above as upon LPAs arrival, there was one staff(S1) with 20 preschool age children, which poses a potential health, safety or personal rights risk to persons in care.

ALLEGATION(S): Staff are yelling at day care children Staff are inappropriately punishing day care children Staff do not prevent day care children from being bullied by other children INVESTIGATION FINDINGS: On July 29, 2026 at approximately 11:04am, Licensing Program Analyst (LPA) Tapia-Mandujano arrived at the facility main entrance to conduct an unannounced inspection to the facility to deliver investigation findings in response to the above allegation that was made to the department on May 20th, 2026. LPA met with Site Director, Y.M.Z. and discussed the purpose of the visit. Facility is a preschool with a toddler component. Present during walk through were Site Director and seven staff supervising a total of 57 children (11 toddlers and 46 preschoolers). All adults present are fingerprint cleared and associated to the facility. During today’s visit, LPAs conducted a health and safety inspection and interviewed site director. Continued on Page 2... This requirement is not being met as evidence by: Based on observation, interview, and record review, the facility did not comply with the section cited above as when LPA was conducting physical plant inspection, there was one staff with 22 children and then one staff with 15 children which poses a potential health, safety or personal rights risk to persons in care.

CCR101223(a)(2) · Type B

101223: Personal Rights: " (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs." This requirement is not being met as evidence by: Based on observation, the facility did not comply with the section cited above as LPA observed a child dozing off in a chair that is not appropriate equipment to sleep and a staff (S2) sitting on top of child's cot with a child in arms which is not safe. This poses a potential health, safety or personal rights risk to persons in care.

CCR 101227(a)(1) · Type B

101227: Food Services: " (a) In child care centers providing meals to children, the following shall apply:...Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20,...Requirements for Meals, for the age group served." This requirement is not met by evidence by: Based on interviews, observations, and record review, the facility did not comply with the section cited above as the facility is not meeting the nutritional guidelines for the amount of food components requirement for lunch meals based on age group. This poses a potential health, safety or personal rights risk to persons in care.

ALLEGATION(S): Staff are using inappropriate forms of punishment for day care children. Staff are not providing a comfortable environment for day care children. INVESTIGATION FINDINGS: On June 24, 2025, at approximately 1:30 p.m., Licensing Program Analyst (LPA) Zeynep Basak conducted an unannounced inspection to finalize the complaint received on June 4, 2025. Upon arrival, LPA met with the Director, Y.L., and the Assistant Director Yun Zhang, and explained the purpose of the visit after being granted to entry. During today’s visit, LPA observed 10 staff members, including the directors, and 43 preschool children in attendance. LPA verified the staff member's criminal background clearance through the Guardian website. The center's operation hours are Monday through Friday from 8:00 a.m. to 6:00 p.m. As part of the complaint investigation, LPA obtained pertinent documentation, including LIC 500 and LIC 9040, reviewed records, and conducted interviews with staff members.
