ALLEGATION(S): -Staff did not prevent a daycare child from causing harm to other daycare children. -Unqualified staff are providing care and supervision. INVESTIGATION FINDINGS: On July 2, 2026, at approximately 9:40AM, Licensing Program Analysts (LPAs) Tse and Tran conducted an unannounced complaint investigation visit at the facility. LPAs met with Director, Y.M.L., and explained the purpose of the visit. Present during the visit were 11 staff members and 40 children (28 preschool age children and 12 toddlers). During today's visit, LPAs conducted site observations and record review. During the course of the investigation, LPAs conducted site observations, record review, and interviews with relevant parties. Based on site observations, LPAs observed there to be sufficient staff to meet ratios for both the toddler and preschool components.
