Posting licensing report by child care facility or homeduration of posting; civil penalty for failure to comply; reports to be provided to parents or guardian of each child receiving services: (2) Upon enrollment of a new child in a facility, the licensee shall provide to the parents or legal guardians of the newly enrolling child copies of any licensing report that the licensee has received during the prior 12-month period that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as set forth in paragraph (1) of subdivision (a) of Section 1596.893b. This requirement is not met as evidenced by: Based on interviews and records review, the licensee did not comply with the section cited above where LIC 9224( Acknowledgement of Receipt of licensing reports) is not signed by the parents of 3 children( C1, C2, and C4) out of 6 children for not sharing the Type A citation report that was issued on 04/05/2024, which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs.(1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met us evidenced by: Based on interviews, children were left alone with an unqualified staff (S1) at the facility on March 25, 2024. This poses an immediate risk to the health, safety and personal rights of children in care.
Health-Related Services; The licensee shall immediately notify the child's authorized representative if the child becomes ill or sustains an injury more serious than a minor cut or scratch. The licensee shall obtain specific instructions from the authorized representative regarding action to be taken. This requirement was not met us evidenced by: Based on interviews, parents were not informed immediately regarding an injury to a child on March 25, 2024. This poses a potential risk to the health, safety and personal rights of children in care.
