ALLEGATION(S): Staff pushed child Staff handled child roughly INVESTIGATION FINDINGS: On July 23, 2025, at approximately 11:45 am, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced inspection and met with Lead teacher, C.N. The purpose of the inspection was explained and was to deliver complaint findings for the above allegations. Present in the facility were Lead teacher, 5 staff caring for a total of 23 preschool children. Facility is operating within capacity. During today's inspection, LPA inspected the facility for health and safety hazards and conducted interviews. During the course of the investigation, LPA inspected facility, conducted interviews, conducted observations, and reviewed pertinent documentation provided. Based on evidence obtained, it was determined that the allegations, Staff pushed child and Staff handled child roughly, are determined to be UNSUBSTANTIATED. Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Upon receipt of this report, Facility shall post the Notice of Site Visit for 30 consecutive days. Failure to maintain posting as required will result in an immediate $100 civil penalty. This report is public and can be reviewed. Exit interview was conducted and report was reviewed with Lead teacher, C.N. Appeal rights were provided. Unsubstantiated Estimated Days of Completion:

(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test. This requirement is not met as evidenced by: Based upon record review, the facility did not test for lead until 2/25/2023. This posed a potential health, safety, or personal rights risk to the persons in care.
