101229 Responsibility for Providing Care and Supervision: (a) "The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation." This requirement was not met as evidenced by: Based on interview and record review, the facility did not comply with the section cited above as facility self-reported that there was a child left alone outside during transition back to the classroom from the outdoor play yard, which poses a potential health, safety or personal rights risk to persons in care.

ALLEGATION(S): Staff handled children in care in a rough manner. INVESTIGATION FINDINGS: On January 26th, 2026 at approximately 9:35am, Licensing Program Analysts (LPA) Tapia-Mandujano conducted an unannounced inspection to conclude the pending complaint investigation. LPA met with Site Supervisor, Roxana Pena regarding the above allegation and explained the purpose of the visit. Present in the facility are Site Supervisor and 10 teachers caring for 47 preschool age children. All adults working at the facility have fingerprint clearance through the Redwood City School District. During today’s inspection, LPA and Site Supervisor toured the facility for health and safety hazards. Complaint was received by the Department on November 24th, 2025. During the course of the investigation, interviews were conducted with staff, children, and involved parties. LPA also received and reviewed relevant documents. Based on interviews and information that was gathered, LPA determined that based on the information obtained there is insufficient evidence to state that Staff handled children in care in a rough manner. Per interviews with staff, no observations were made of a staff handling a child in a rough manner. Per interviews with children, no child reported being handled in a rough manner. Although the allegation above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the above allegation is UNSUBSTANTIATED. Notice of site visit was given and must remain posted for 30 days. Failure to maintain postings as required will result in an immediate $100 civil penalty. (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above. In classroom #1, LPA observed knives stored in a drawer without a child safety lock, which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR 101238(g)(1) · Type B

(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. (1) Storage areas for poisons shall be locked. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above. In classroom #3, LPA observed poisons stored in a classroom cabinet without the safety lock in place or locked, which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR 101212(d) · Type B

101212(d): Reproting Requirements: "Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours." This Requirement is not met by: Based on interview and record review, the facility did not comply with the section cited above as the facility failed to call in the incident that occurred on 3/20/25 until 3/25/25 which poses a potential health, safety or personal rights risk to persons in care.

ALLEGATION(S): Staff did not provide adequate supervision resulting in child sustaining an injury INVESTIGATION FINDINGS: On April 15th, 2025 at approximately 11:40am, Licensing Program Analysts (LPAs) Tapia-Mandujano and Wadhwa conducted an unannounced inspection to conclude the pending complaint investigation. LPAs met with Site Supervisor, Roxana Pena regarding the above allegation and explained the purpose of the visit. Present in the facility are Site Supervisor and 13 teachers caring for 48 preschool age children. All adults working at the facility have fingerprint clearance through the Redwood City School District. During the today’s inspection, LPAs and Site Supervisor toured the facility for health and safety hazards and conducted two staff interviews. Complaint was received by the Department on 02/28/25. During the course of the investigation, interviews were conducted with staff at the facility. Interviews with involved parties were conducted. As well as relevant documents were gathered. 101700.3 California Lead Action Level at Child Care Centers (b)(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidenced by: Based on observation, interview, and record review, the facility did not ensure to maintain a level to be under 5.5 ppb at fixtures used for drinking and food prepration. This poses a potential health, safety or personal rights risk to persons in care.

101219(f): Admission Agreements: "The licensee shall comply with all terms and conditions set forth in the admission agreement." This requirement is not met by: Based on interviews and record review, the facility did not comply with the section cited above as staff are not following the operational hours set forth by the admission agreement, which poses a potential health, safety or personal rights risk to persons in care.

ALLEGATION(S): Staff do not maintain proper teacher-child ratios INVESTIGATION FINDINGS: On June 14th at approximately 11:30am, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced, complaint investigation and met with Site Supervisor, Roxana Pena regarding above allegations and explained the purpose of the visit. LPA and Site Supervisor toured the facility for health and safety hazards and LPA conducted Staff interviews. Complaint was received by the Department on 04/18/23. Present in the facility are Site Supervisor and 10 teachers caring for 56 preschool age children. All adults working or living in the facility have fingerprint clearance and are associated through the School District. During the course of the investigation, interviews were conducted with parents and staff, and relevant documents were gathered. LPA also did unannounced inspections in three different occasions. Per interviews, documentation, and observations, it was determined that staff were maintaing proper teacher-child ratios. Although the allegation above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the above allegations are UNSUBSTANTIATED. Upon receipt of this report, Licensee shall post the Notice of Site Visit. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain postings as required, will result in an immediate $100 civil penalty. This report is public and can be reviewed. After today’s visit, an exit interview was conducted, report was reviewed and copy was provided to Site Supervisor, Roxana Pena. Unsubstantiated Estimated Days of Completion:
