ALLEGATION(S): Staff do not provide adequate care and supervision of the daycare children Staff yell at the daycare children INVESTIGATION FINDINGS: On November 22, 2024 @ approx. 12:15pm, Licensing Program Analyst (LPA) Maria Olguin-Leon, conducted an unannounced visit to close the complaint investigation into the above allegations and met with Director Roseanne Bautista Abad. Present during today’s visit was Director, 1 staff and 7 children. During the Investigation, LPA conducted file reviews, interviewed staff, parents, and conduct three unannounced visits. During the course of the investigation, LPA was unable to determined staff do not provide adequate care and supervision of the daycare children and staff yell at the daycare children. Based on investigation and information gathered, the preponderance of evidence standard has not been met. Although the above allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations did or did not occur, therefore, the above allegations are UNSUBSTANTIATED. This report and exit interview were conducted with Director, Roseanne Bautista Abad. Notice of Site Visit shall remain posted for 30 days. 101218.1 Admission Procedures and Parental and Authorized Representative's Rights (e) At the time of acceptance of each child into care and for all children in care on August 7, 2002, the licensee shall give each parent or authorized representative a copy of the Notification of Parents' Rights (LIC 995 [8/02]) and the LIC 995E, Caregiver Background Check Process form. (1) The licensee shall request the child’s parent or authorized representative to sign and date the acknowledgement-of-receipt statement at the bottom of the LIC 995 (8/02). The bottom portion of this form must be kept in the child's file as proof that the parent or authorized representative has been notified of his or her rights by the child care center and received a copy of the LIC 995E, Caregiver Background Check Process form. 1596.841 Current roster of children provided care in facility required Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. 101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
