(A) Staff shall place infants up to 12-months of age on their backs for sleeping. This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above by having three out of three infants under the age of 1 napping on their sides or stomach without a completed LIC9227, which poses a potential health, safety or personal rights risk to persons in care.
