ALLEGATION(S): Staff do not keep the facility free of pests. INVESTIGATION FINDINGS: On 07/24/2026 at 9:45am, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced Complaint Investigation Visit for the above allegation of a physical plant violation. LPA met with facility representative, James Kim. Also present at the time of today’s visit is 12 staff and 49 day care children in 3 classrooms. This agency has investigated the complaint allegation that the staff do not keep the facility free of pests. During the course of the investigation, LPA Uribe conducted interviews, obtained relevant documents, and made observations within the facility. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur, therefore the allegation is unsubstantiated. ALLEGATION(S): Staff handle children roughly Staff yell at children INVESTIGATION FINDINGS: On 6/2/2025 at 10:19am Licensing Program Analyst (LPA) Morgan Pringle met with Director M.F. to conclude an investigation for a complaint that was received alleging a child's personal rights had been violated. Present during LPAs visit were fifty-one (51) preschool age children, and twelve (12) additional staff members. During LPA's investigation interviews were conducted and facility files were collected. Based on conflicting evidence collected, LPA determined although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegation is UNSUBSTANTIATED. (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as one staff member needs to obtain proof of immunization against Measles (MMR) & Influenza which poses/posed a potential health, safety or personal rights risk to persons in care.
