ALLEGATION(S): Staff do not practice sanitary diapering procedures Staff do not adequately maintain the infant outdoor play space Staff do not provide adequate supervision to infants in care INVESTIGATION FINDINGS: On 01/09/2026 at 9:15am, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced Complaint Investigation Visit for the above allegation of a personal rights violation. LPA met with site director, Dana Tanaka. Also present at the time of today's visit is 3 staff and 8 day care children. This agency has investigated the complaint allegation that staff do not practice sanitary diapering practices. During the course of the investigation, LPA Uribe conducted interviews and made observations within the facility. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Notice of Site Visit was given and must be posted for 30 days. Exit interview conducted and report was reviewed with the site director, Dana Tanaka.
