Responsibility for Providing Care and Supervision (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement was not met as evidenced by: Based on observation, the licensee did not comply with the above by having 2 preschool children in a bathroom without the supervision of a teacher. This poses a potential risk to the health and safety of children in care.

ALLEGATION(S): -Staff did not meet child's diapering needs INVESTIGATION FINDINGS: On 12/3/2025, at approximately 11:40AM, Licensing Program Analyst (LPA) Jonathan Tse conducted an unannounced complaint investigation visit at the facility. LPA met with Director, S.H., and explained the purpose of the visit. Present during the visit was the Director, 8 staff members, and 44 preschool age children. During the course of the investigation, LPA conducted site observations, record review, and interviews with relevant parties. The facility denied the allegation. LPA did not observe direct evidence to support or deny the above allegation. The preponderance of evidence standard has not been met, therefore the above allegation is found to be unsubstantiated at this time. No deficiencies were cited during today's visit on 12/3/2025. Appeal rights were provided and explained. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, S.H. Teacher-Child Ratio (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance... This requirement was not met as evidenced by: Based on site observations, interview, and record review, the facility did not comply with the above by having teachers supervise more than 12 children at a time without an aide or another teacher present. This poses a potential risk to the health, safety, or personal rights of persons in care.

HSC 1596.7995(a)(1) · Type B

(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in five out of eight staff files which poses a potential health, safety or personal rights risk to persons in care.

Administration of Child Day Care Licensing (b) (1) ... licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training p... shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement was not met as evidenced by: Based on review of 7 files it was found that 5 staff files were missing mandated reporter trainings. This poses a potential health and safety risk to children in care.

This requirement was not met as evidenced by: Based on interviews and record review, the facility self-reported a child was left unattended and unsupervised for 1-2 minutes. This poses an immediate Health, Safety and/or Personal Rights risk to children in care.

ALLEGATION(S): Facility is operating out of ratio. INVESTIGATION FINDINGS: On April 4th, 2024, at approximatly 3:30 PM, Licensing Program Analyst (LPAs) Janet Gil and Kassandra Medrano conducted an unannounced inspection in order to deliver findings on the complaint investigation for the above allegation. LPAs Gil and Medrano met with the director, M.B. to discuss complaint allegations findings. Based on LPAs record reviews, and interviews which were conducted. The allegation may have happened or is valid, however there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Exit interview conducted and a copy of this report and appeal rights were reviewed and provided to director, M.B. Notice of Site visit was observed to be posted and shall remain posted for 30 days. No deficiencies were issued today under Title 22 Division 12 of the California Code of Regulations. NOTICE OF SITE VISIT WAS GIVEN AND SHALL REMAIN POSTED FOR 30 DAYS. 101700.3 California Lead Action Level at Child Care Centers (b)(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidenced by: Based on results received from the certified water samplers, the facility exceeded 5.4 ppb of lead in their water source. This poses a potential health, safety, or personal rights risk to persons in care.

101219(f) Admission Agreements…The licensee shall comply with all terms and conditions set forth in the admission agreement. This requirement was not met as evidence by: Based on observations, interviews and record review, the facility’s admission agreement states the involvement of a child’s family during child’s classroom transition. The facility moved a child from their assigned classroom to an older classroom without prior guardian consent, notice and/or involvement. Facility did not follow admission agreement or parent handbook. This poses a potential health, safety or personal rights risk to children in care.
