ALLEGATION(S): Unqualified staff are left to supervise children alone Staff left children outside unsupervised INVESTIGATION FINDINGS: On 07/17/2025, at 11:30am, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced visit for the purpose of investigating a complaint for the above allegation of qualifications violation. LPA met with director, Hee Sung Kim. Present during the time of today’s inspection was 31 children and 11 staff. This agency has investigated the complaint allegation that unqualified staff are left to supervise children alone. During the course of the investigation, LPA Uribe conducted interviews and collected relevant documents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Notice of Site Visit was given and must be posted for 30 days. Exit interview conducted and report was reviewed with the director, Hee Sung Kim. 101229(a)(1) Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time except as specified in sections 101216.2(e)(1) and 101230(c)(1). This requirement was not met as evidenced by interviews and record review conducted. This poses an immediate risk for the health and safety of children in care. WHILE IN CARE, A CHILD WAS LEFT OUTSIDE THE FACILITY AND OBSERVED TO BE STANDING OUTSIDE A CLOSED DOOR ON THE PLAYGROUND ALONE.

(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above as several containers of disinfecting wipes and a box of magic erasers were observed being stored in a manner that made them accessible to children which poses an immediate health, safety or personal rights risk to persons in care.
(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above as a minor child (12 years of age) was present today which poses/posed a potential health, safety or personal rights risk to persons in care.
(e) In centers where the licensee chooses to handle medications: (1) All prescription and nonprescription medications shall be centrally stored in accordance with the requirements specified below: (B) Each container shall have an unaltered label. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above as there were 2 medications (an inhaler and an EpiPen) that were not kept in their original packaging (and were also expired) which poses a potential health, safety or personal rights risk to persons in care.
(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above as there were scissors and a knife in kept in a drawer in the kitchen accessible by children which poses a potential health, safety or personal rights risk to persons in care.
(1) All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20, (Revised January 1, 1990) Requirements for Meals, for the age group served. All food shall be selected, stored, prepared and served in a safe and healthful manner. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above as there were two gallons of 2% milk for children's consumption kept in the refrigerator which poses a potential health, safety or personal rights risk to persons in care.
101238(a)(1) Buildings and Grounds. The licensee shall take measures to keep the center free of flies, other insects and rodents. This requirement was not met as evidenced by: Based on observation, at 12:34pm LPA Plumboy observed a roach in the facility and informed the center director. Insects in the facility poses a potential health and safety risk to children in care.
