CCR 101239(h) · Type B

This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above by changing a child bathroom to a staff bathroom, thereby no longer meeting the toilet ratio of 1:15, which poses a potential health, safety or personal rights risk to persons in care.

CCR 101229(a)(1) · Type B

(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above by allowing a preschool-age child to leave the napping area and enter the children's restroom without the supervision of a teacher, which poses a potential health, safety or personal rights risk to persons in care.

CCR 101212(c) · Type B

The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space. Such structural changes shall include, but not be limited to, room additions. This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above by changing the size of the infant outdoor play area without reporting changes to the Department, which poses a potential health, safety or personal rights risk to persons in care.
