ALLEGATION(S): Licensee takes children to an unsafe environment. INVESTIGATION FINDINGS: On 02/04/2026, at 08:40AM, Licensing Program Analyst(LPA) Mandeep Kaur and Licensing Program Manager(LPM), Mireya Flores met with Licensee's assistants (S1 & S2) for an unannounced follow up complaint investigation. Upon arrival, during today's investigation, LPA and LPM observed 10 children (Three infants and 7 preschool age) with Licensee's assistants (S1& S2)) inside the home. At 08:53AM, Licensee, Gladys Rocha arrived at the facility and LPM explained the Purpose of today's investigation: deliver investigation findings. LPA conducted observations, interviewed Licensee, staff, random parents and random children during the complaint investigation. LPA toured indoor and outdoor areas of the facility during investigation. Based on interviews and observations during the investigation process, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED. **Continue on next page** Unsubstantiated Estimated Days of Completion:

(c) The family day care home shall maintain documentation of the required immunizations or exemptions from immunization, as set forth in this section, in the person's personnel record that is maintained by the family day care home. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in one out of one staff(S1) do not have copy of Tuberculosis (TB)test results on file, which poses a potential health, safety or personal rights risk to persons in care.

Staffing Ratio and Capacity: (d) For a Large Family Child Care Home, the maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home, including children under age 10 who reside at the licensee's home and the assistant provider's children under age 10, shall be either: (2) More than twelve and up to fourteen children only if the criteria in Section 1597.465 of the Health and Safety Code are met. This requirement was not met as evidenced by: Based on observation, upon arrival at 09:30AM, LPA observed 12 children (8 preschool and 4 infants) present with one assistant, K.X. D.S. during today's inspection, which poses an immediate Health, Safety, or Personal Rights risk to persons in care.

Criminal Record Clearance :(d)All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing, or volunteering in a licensed facility:(1)Obtain a California clearance or a criminal record exemption as required by the Department or,,, This requirement was not met as evidenced by: Based on observations,records review and interview, three adult assistants(S1, S2 &S3) do not have criminal record clearance on file.

ALLEGATION(S): Facility is operating over capacity. Licensee does not reside at the family child care home. INVESTIGATION FINDINGS: On 10/22/2025, Licensing Program Analyst(LPA) Mandeep Kaur conducted an unannounced follow up complaint investigation. At 09:24 AM, LPA was authorized entry by Assistant, Daniela Munoz(S2) and explained the reason for the visit: deliver investigation findings. Present was Assistant(S1), Assistant(S2), an adult/chef (A2) with 11 children in care: 4 infants and 7 preschool age children. Later, an adult (A1) arrived at the facility and LPA explained the reason of the today's investigation. At 09:49AM, Licensee, Gladys Rocha arrived at the facility and the reason of the today's investigation was explained. LPA conducted complaint investigation comprising of interviews, observations, and records review. LPA toured indoor and outdoor areas of the facility during investigation.
