ALLEGATION(S): Daycare child sustained multiple injuries due to staff neglect or physical abuse. INVESTIGATION FINDINGS: On 03/19/2026, Licensing Program Analyst (LPA) Mandeep Kaur conducted an unannounced follow up complaint investigation. LPA met with Licensee, Fethya Mohammed and explained the purpose of today's investigation: deliver investigation findings. During today's investigation, present were Licensee's Assistant(S1) and 12 children(2 infants and 10 preschool age). LPA conducted complaint investigation comprising of observations, Licensee's interview, staff interviews, random parents interviews, children interviews and attempted interviews with children, records review and evidence gathered. Based on interviews, on 01/28/2026, a staff(S2) observed a scratch on a child(C1). Based on evidence gathered and interviews, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. LPA was greeted by Licensee Fethya Mohammed, and one staff. Present were two infant( under two years old), four toddlers and one school aged child. Complainant alleges that Licensee/ Provider locked day care child in the garage. Day care child is offered money to do the work in the day care, Provider leaves the children alone with a babysitter and Provider is not providing adequate supervision. LPA obtained information from the interviews that were conducted with the Licensee, parents, and other parties involved. LPA, also, reviewed supporting documentation, which included the Facility Roster and police report, Based on the information obtained, although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. Licensee confirms the dissociation. Sebrir Hussen began employment on 9/1/2021, which poses an immediate risk to the Health, Safety or Personal Rights to the children in care
MANDATED REPORTER TRAINING. [...] a person who, on January 1, 2018, is a licensed child care provider [...] shall complete the mandated reporter training provided [...] and shall complete renewal mandated reporter training every two years LPA observed that Licensee did have not current Mandated Reporter Training.It expired on 3/2020 and her husband Khamal Mohanned does not have training on file. This poses a potential risk to children's health and safety of children in care.
IMMUNIZATIONS. The licensee shall document each child's immunizations [...] and maintain such documentation for as long as the child is enrolled. This requirement includes updating each child's PM 286 (6/95) when the child is due to receive required immunizations after enrollment in the family day care home. This requirement is not met as evidenced by: LPA was unable to review immunizations record for C1 This poses a potential risk to the health & safety of children.t
Personnel Requirements. The licensee and other personnel as specified, shall complete training on preventive health practices including CPR and first Aid. Licensee CPR/1st Aid certificate expired in 4/2021. This poses a potential risk to the Health, Safety, or Personal Rights of children in care.
