(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as 3 of 3 staff files reviewd by LPA did not have current Mandated Reporter Training (Child Care Providers) Certificates on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

ALLEGATION(S): Staff are operating over ratio Unqualified staff left alone to supervise children INVESTIGATION FINDINGS: On 3/12/24 at, 9:15 AM Licensing Program Analyst (LPA) Michael Mathew conducted an unannounced inspection to conclude a complaint investigation and completed a COVID-19 pre-screening questions prior to entering the facility. LPA met with Director R.A. and advised her the purpose of the inspection. Director provided LPA a tour of the facility inside and out. There were 17 children and 2 staff in care at the time of the inspection. Allegation: Staff are operating over ratio. During the investigation, LPA interviewed Reporting party, staff members, and parents. Based on interviews conducted, there is not a preponderance of evidence to prove the alleged violation did or did not occur, meaning the allegations may have happened or are valid. Therefore, the allegations are deemed UNSUBSTANTIATED. Report was reviewed and a notice of site visit was given and must remain posted for 30 days. Exit interview was conducted with director R.A. 101700.3(b)(1)A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. Sample ID PA C faucet had a Lead Action Level of Exceedance Response of more than 5.5ppb. This poses a potential health, safety and personal rights risk to children in care.
