HSC 1596.8662(b)(1) · Type B

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as Licensees and assistants all do not have current Mandated Reporter "Child Care Providers" certificates, which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR 102416(c) · Type B

(c) The licensee and other personnel as specified shall complete training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as Licensee and 1 of 2 assistants no longer have valid Pediatric CPR/First Aid certifications (expired 10/2025), which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR 102425(j)(2)(D) · Type B

Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following: This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as two (2) infants in care did not have 15-minute sleep checks documented, which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR 102416.5(d)(2) · Type B

(d) For a Large Family Child Care Home, the maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home, including children under age 10 who reside at the licensee's home and the assistant provider's children under age 10, shall be either: (2) More than twelve and up to fourteen children only if the criteria in Section 1597.465 of the Health and Safety Code are met. This requirement is not met as evidenced by: Based on observations and record review, the licensee did not comply with the section cited above as 14 children were present during today's inspection. One child was not at least 6 years of age as required by Licensing regulations, which poses/posed a potential health, safety or personal rights risk to persons in care.
