ALLEGATION(S): 1. Facility staff caused injury to child while in care 2. Staff did not notify child's authorized person of serious injury in a timely manner INVESTIGATION FINDINGS: On 12/16/25, Licensing Program Analyst(LPA) Anna Morales conducted an Unannounced Subsequent Complaint investigation to deliver the findings for the above allegations. LPA met with Annaleen Encina, Operations Manager. LPA conducted a tour of each of the classrooms and observed that the facility was operating in compliance with teacher to children ratio requirement. During the course of the investigation, LPA Morales interviewed children, staff, reviewed supporting documentation and toured the facility. Children interviewed stated that they have never seen any of the teachers hit or grab anyone by the hands. Children stated that they like coming to school and like all of their teachers. ALLEGATION(S): 1. Facility did not meet child's needs. INVESTIGATION FINDINGS: Licensing Program Analyst (LPA) Anna Morales conducted an unannounced subsequent complaint investigation today and met with Director C.P. to deliver investigation finding for the above allegation. On 3/14/25, Licensing Program Analyst(LPA) Anna Morales conducted an initial complaint investigation, interviewed the Director and obtained Classroom/Children's roster. On 3/18/25, LPA Anna Morales interviewed five parents who stated that they have no concerns and the facility is meeting their child(ren) needs. Parents stated that the staff are good at communication and their child(ren) are enjoying the school. On 3/19/25, LPA interviewed three staff and four children. Staff stated that during lunch time, the children sit together in their assigned tables, and place their food from their lunch boxes onto a plate. ALLEGATION(S): 1. Day care child sustained an unexplained injury while in care due to lack of staff supervision. 2. Staff member handled day care child in a rough manner while in care. 3. Staff did not report an incident involving day care child in care as necessary. 4. Staff member encouraged day care child to withhold information about an incident from their responsible party. INVESTIGATION FINDINGS: Licensing Program Analyst (LPA) Anna Morales made an unannounced follow-up complaint investigation inspection. LPA met with Center Director, C.P. The purpose of today's inspection was to deliver the findings for the above allegations. LPA Morales interviewed staff, parents, and other parties involved, observed the Classrooms, as well as obtained copies of pertinent information. Throughout the investigation process, it was found the above allegations are UNSUBSTANTIATED; A finding that is unsubstantiated means although the allegations may have happened or is valid, the preponderance of evidence does not prove it. Exit interview conducted and copy of this report was reviewed with the Director C.P.
