(1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: On 11/04/24, a child (C1) was left unattended in the playground. This posed an immediate risk to the health, safety, and personal rights to persons in care.

ALLEGATION(S): Due to neglect daycare child sustained a UTI twice while in care Staff failed to meet child's hygiene needs INVESTIGATION FINDINGS: Licensing Program Analyst (LPA) Marilou Monico met with Site Director, L.Y., for an unannounced complaint inspection to deliver investigation findings. During the investigation process, LPA interviewed staff and parents, obtained medical reports and other documents. Based on the available evidence, it is concluded that although the allegations noted on this complaint may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. The allegations are thus UNSUBSTANTIATED. There were no deficiencies cited. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS. Unsubstantiated Estimated Days of Completion:
