ALLEGATION(S): Staff did not meet the needs of the daycare children. Staff mishandled/mistreated the daycare children. INVESTIGATION FINDINGS: On 01/15/2026, Licensing Program Analyst (LPA) Mandeep Kaur met with Director, Gita Nezamfar for an unannounced follow up complaint investigation. Purpose of today's investigation: deliver investigation findings. LPA conducted complaint investigation comprising of observations, random staff interviews, random parents interviews, random children interviews, records review and evidence gathered. Based on interviews, observations, records review and evidence gathered during the investigation process, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED. No deficiency issued during today's investigation. Appeal rights were provided. Exit interview conducted and report was reviewed with Director, Gita Nezamfar. INVESTIGATION FINDINGS: Licensing Program Analyst (LPA) Mandeep Kaur met with Assistant Director Stephanie Gates, for an unannounced follow up complaint investigation. Purpose of today's investigation: deliver investigation findings. LPA reviewed the staff files, a child(C1) file, attempted to interview children, interviewed staff and parents during the investigation. LPA toured the inside and outside of the facility during investigation. Director, Gita Nezamfar joined later during today's investigation and purpose of the today's investigation was explained: deliver investigation findings. LPA interviewed two (2) staff during today's investigation including Director. Incident reports for the child (C1) were provided via email to LPA before the conclusion of today's investigation. Based on interviews, observations, records review and evidence gathered during the investigation process, it is concluded that although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. Personal Rights: The licensee shall ensure that each child is accorded the following personal rights: (3)To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met as evidenced by: a preschool teacher(S1) slapped one preschool child(C1) on left cheek on 02/22/24 during rest time. This poses an immediate risk to the health, safety and personal rights of children in care.
